The printing size is not the same that the pixel size of the image. It depends on resolution. For example, a low resolution pic, about 30 pixels/cm. A cd cover is 12,1x12,1 cm, so 12,1x30=363 pixels. Very low quality.
I use high spixel sizes for cd covers, about 2500-3000 pixels height.
Then you make the cover, and then you have 2 modes to print the cover at correct size (in Photoshop)
One:
Use the option Image size in the Image menu. Deselect resampling image, you will see that the pixel sixe boxes lock. Then, you can put manually the printing size, and you will see the resolution changing.
Two
Print with preview option, in archive menu. You can use this option to put manually the size in the page at printing, and you can preview the page.
I hope you understand me, my english is not very good _________________
